CINCO is part of the
Community Services Department at Glendale Adventist
Medical Center which offers the community health
education, Heart Disease and Diabetes Screening
programs, Bereavement Services and Training. Below is a
list of programs and contact information.
CARDIOVASCULAR COMMUNITY HEALTH SCREENING
Heart Disease Risk Assessment
The American Heart Association and the Centers for
Disease Control and Prevention (CDC) recommend that
those at moderate risk for heart disease be tested for
CRP (C-Reactive Protein) in addition to cholesterol. CRP
has recently been discovered as a new indicator for
heart disease. One out of four people who have normal
cholesterol levels may still have high CRP levels, and
therefore may be at risk for heart disease. In 2003, the
Heart and Vascular Institute at GAMC was awarded a
$615,000 grant from the UniHealth Foundation to screen
members of the community for CRP.
OUR GOAL
To pre-screen 20,000 community residents over the next
two years, identify those at risk for
heart disease, and refer them to treatment to mitigate
that risk.
HEALTH SCREENING COMPONENTS
ALL OF THE FOLLOWING IS FREE:
PRESCREENING
Includes a brief, self-scoring Heart Health Survey. If
participants score 8 or greater on the survey,
participants fill in their contact information on the
back of the survey and a representative will contact
them within a week to schedule a clinical screening
appointment.
CLINICAL SCREENING
Includes symptoms assessment, blood pressure check,
blood draw for cholesterol and CRP, and health
counseling by a health care professional. Lab results
will be mailed to participants. Participants with
abnormal lab results will be contacted for further case
management.
CASE MANAGEMENT
Includes referral to participant’s primary care
physician (PCP) for further treatment. If participant
does not have a PCP, a list of physicians will be
provided which the participant can choose from.
Insurance enrollment is also provided for those with no
health insurance. Participants will also be invited to
attend free health education classes in heart disease
prevention.

DIABETES RISK ASSESSMENT SCREENING PROGRAM
OUR GOAL
To pre-screen as many community residents of Glendale
and Northeast Los Angeles while funding exists in order
to identify those who are diabetic and are unaware of
it, as well as those who are at risk for type II
diabetes. Ultimately, we would like to refer these
diagnosed patients to treatment and education in order
to make lifestyle changes in order to live longer,
healthier, happy lives and to prevent complications.
HEALTH SCREENING COMPONENTS
ALL OF THE FOLLOWING IS FREE:
PRESCREENING
Includes a brief, self-scoring Diabetes Risk Assessment
Survey (assistance is available). If participants score
10 points or more on the survey, a representative will
contact them within two weeks to schedule a clinical
screening appointment.
CLINICAL SCREENING
Includes symptoms assessment, a 2-hour glucose tolerance
test (not provided at most doctor’s offices), and health
counseling and education by a health care professional
(either a Certified Diabetes Educator and/or Medical
Director/Endocrinologist). A copy of the lab results
will be mailed to all participants. Participants with
abnormal lab results will be contacted for further case
management.
CASE MANAGEMENT
Includes a referral to the participant’s primary care
physician (PCP) for further treatment. If participant
does not have a PCP or health insurance coverage, a list
of medical care clinics will be provided which the
participant can choose from. A referral will accompany
the patient to the clinic of their choice. The list of
medical care clinics always consists of Federally
Qualified Health Centers (FQHC), which provides no-cost
medical care, or low-cost community based clinics, which
offer discounts. Insurance enrollment is also provided
for those with no health insurance.

HEARTS N' HEALTH PROGRAM
The Hearts N’ Health Program is a free community health
service, offered by Glendale Adventist Medical Center,
Community Services and currently funded by Unihealth
Foundation. The Hearts N’ Health Program offers health
activities that will reduce heart disease and improve
the health for the members of the Armenian Community.
Community members do not have to have medical insurance
to qualify for this service.
PROGRAM GOALS
· Improve cardiac health
· Decrease heart disease
· Lower cholesterol levels
· Increase Armenian Community participation in health
promotion activities
HISTORY
This Promatoras Comunitarias-based program has trained
more than two-dozen local Armenian-speaking women to
conduct outreach regarding the prevention of
cardiovascular disease and other related health issues.
These community health educators have conducted more
than 1000 home presentations with between 6000 – 8000
participants.
PROCESS
Presenters/Community Health Educators - A group of women
go out into the Glendale community, in this case the
Armenian Community, where they educate the community
about heart disease, nutrition, and physical activity.
We currently have six presenters, who are also members
of the Armenian community, that provide this community
health service. The presenters follow the standards and
the material covered in their Hearts ‘N Health manuals.
The material is presented in Armenian. Prior to going
out into the community, the presenters are trained by
the outreach coordinator.
Hostesses - Hostesses, usually women, open their houses
to guests and the presenters for the Hearts N’ Health
presentations. Presenters educate guests about heart
disease, nutrition and physical activity. Using the
hostesses’ homes provides a friendly, comfortable
setting for guests, where they are willing to open up
and share their questions.
Manuals - Created by a Registered Dietician. The manual
is translated in Armenian, and is detailed that can
easily be understood by the Armenian Speaking Community.
The manual consists of easy to read tables and charts
explaining a different cholesterol levels, showing a
persons cholesterol test results and risk factors for
heart attacks. The graphics in the manual are very
detailed showing the heart, blockage on the heart, and
different kinds of food to eat and are great for visual
purposes.
Topics being covered - prevention of heart disease, bad
cholesterol vs. good cholesterol, nutrition, proper
eating habits, physical activity and how to live a
healthier lifestyle.

CONSORTIUM OF SAFETY NET PROVIDERS
The Consortium of Safety Net Providers (CSNP) was
established in response to major reductions in the
provision of health care in many Los Angeles County
facilities. These facilities traditionally serve the
indigent and underinsured populations of Northeast Los
Angeles and Glendale. The CSNP
membership consists of an array of public and private
medical providers, including the Los Angeles county
Department of Health Services, and local community based
organizations in Glendale and Northeast Los Angeles.
The CSNP initiated a strategic planning process to
develop and strengthen integrated health systems that
coordinate health services for the uninsured and
underinsured. The overarching goal of the strategic
planning process was to identify, coordinate and expand
existing community resources to develop comprehensive
and integrated health services delivery system for the
underserved. This intensive process occurred over a
six-month period, throughout which, the CSNP created a
comprehensive plan and procured federal Healthy
Communities Access Program (HCAP) funding from the
Health Resources and Services Administration (HRSA).
The ultimate goal of the CSNP for HCAP is to design the
infrastructure necessary to develop integrated health
systems that will increase access and improve the
quality of health care. It will combine the
collaboration between the CSNP through coordinated
referral services and improvements in chronic disease
management, and expanded enrollment in public health
insurance programs. This will be accomplished though the
implementation of an electronic medical record (EMR) and
expanded health promotion, screenings, and education.

This training includes:
Terminology, issues facilitators may face, and the roles
of facilitators concerning interventions with the
bereaved.
The art of communication, loss and the family system,
cultural diversity, spirituality, understanding and
working with bereaved through different types of loss
such as child loss, sudden and traumatic loss, suicide,
homicide, AIDS, and much, much more.
